Registration for Guests and Non-CAOS members at the door will be $20.00
All attendees need to register because we need a firm head count.
Due to the Sunshine Law, we must collect state medical license number and NPI#.
*This banquet is partially funded by grants from pharmaceutical companies. Your NPI will be reported as part of the sunshine act unless you choose to pay out-of-pocket for your dinner.
